The Online Services may include features that allow you to upload Genetic Information and other health information; communicate and share information with other registered users of the Online Services; provide information to and receive information from your health care providers, your laboratories, and other third parties; receive personalized educational resources; donate your information; and provide and receive information relating to participation in clinical trials. The features we make available through the Online Services are provided directly to you and not on behalf of any third party, including your health care providers or laboratories. For example, if we receive information about you from third parties, including your healthcare providers and laboratories, we do so on your behalf. None of the information you provide to us, we receive or collect about you, or we make available to you through the Online Services constitutes “protected health information” under federal health privacy laws, including the Health Insurance Portability and Accountability Act of 1996.

None of the information you provide to us, we collect about you, or we make available to you is subject to HIPAA.

The features that we make available through the Online Services are provided directly to you and not on behalf of any third party, including your health care providers or laboratories.

What Information Do We Collect Through the Online Services?

Information You Provide – We collect any information that you provide when you use the Online Services, including when you:

  • create an account;
  • upload your health information, including Genetic Information;
  • populate your user profile;
  • request or provide information relating to participation in clinical trials;
  • participate in the social networking features of the Online Services (e.g., communicating and sharing your information with other users of the Online Services); or
  • otherwise contact us through the Online Services with a question, comment, or request.

The information that you provide us can include, but is not limited to:

  • your name;
  • your photograph;
  • contact information such as physical address, email address, and phone number;
  • user name, password, and other registration information;
  • demographic information such as your gender;
  • health information, including health history (e.g., cancer type and stage, medications) and any clinical characteristics that may be relevant to your condition;
  • Genetic Information; and information you provide when you contact us.

We collect any information you provide when you use the Online Services.

This section contains examples of situations in which you may provide information to us through the Online Services and the types of information that you may provide.

Information Provided by Others – When authorized by you and on your behalf, we may receive information about you, such as Genetic Information and other health information, from your health care providers, your laboratories, our research partners, and other third parties.

At your request, we may receive information about you from your health care providers, laboratories, and other third parties.

Information Automatically Collected From You – We and our service providers may automatically collect certain technical information from your computer or mobile device when you use the Online Services, such as your Internet Protocol address, your browser type, your operating system, the pages you view on the Online Services, and the pages you view immediately before and after you access the Online Services. This information allows us to recognize you and personalize your experience if you return to the Online Services, and to improve the Online Services and the services we provide. We may collect this information using “cookies”—which are small text files that the Online Services save on your computer using your web browser and access when you return—or similar technologies.

  • connecting you with other users of the Online Services or your healthcare providers, at your request;
  • reviewing your information alone, or in combination with the information of other users of the Online Services, to identify users of the Online Services who fit health or genetic-related criteria specified by Cure Forward (which may be done by Cure Forward on behalf of our pharmaceutical manufacturer partners and/or healthcare providers);
  • identifying clinical trial opportunities for which you may be eligible or that may be of interest to you;
  • providing you with information about the Online Services or required notices;
  • facilitating the provision of your information to and the receipt of your information from your health care providers, at your request and on your behalf;
  • providing personalized educational resources;
  • delivering marketing communications, promotional materials, or advertisements that may be of interest;
  • allowing us to improve the Online Services and the services we provide, such as by better tailoring our services to our users’ needs and preferences;
  • generating and analyzing statistics about use of the Online Services;
  • detecting, preventing, and responding to fraud, intellectual property infringement, violations of our Terms of Use, violations of law, or other misuse of the Online Services;
  • purposes disclosed to and authorized by you through just-in-time notifications on the Online Services; and as otherwise authorized by you.

You may choose to disclose your information to friends and/or family members, healthcare providers, and/or other third parties through means other than the Online Services (e.g., disclosing information in person). Please be aware that this Privacy Policy applies only to disclosures by Cure Forward of information collected through the Online Services. If you share your information to third parties outside the Online Services, that activity is not covered by this Privacy Policy.

This Privacy Policy does not apply to any information you disclose outside of the Online Services (e.g., in person).
